What is a coronavirus?
This virus belongs to a family of viruses known as coronaviruses. Named for the crown-like spikes on their surfaces, they infect mostly bats, pigs, and small mammals. But they mutate easily and can jump from animals to humans, and from one human to another.
Seven strains are known to infect humans, including this new virus, causing illnesses in the respiratory tract. Four of those strains cause common colds. Two others, by contrast, rank among the deadliest of human infections: severe acute respiratory syndrome, or SARS, and Middle East respiratory syndrome, or MERS. This new virus is called severe acute respiratory syndrome coronavirus 2, or Sars-CoV-2. The disease it causes is called Covid-19. (The number stands for 2019, the year it emerged.) (2)
Multiple variants of the virus that causes COVID-19 are circulating globally: (32)
- The United Kingdom (UK) identified a variant called B.1.1.7 with a large number of mutations in the fall of 2020.Global Variant ReportsView a global map showing reports of variants in each country.This variant spreads more easily and quickly than other variants. In January 2021, experts in the UK reported that this variant may be associated with an increased risk of death compared to other variant viruses, but more studies are needed to confirm this finding. It has since been detected in many countries around the world. This variant was first detected in the US at the end of December 2020.
- In South Africa, another variant called B.1.351 emerged independently of B.1.1.7. Originally detected in early October 2020, B.1.351 shares some mutations with B.1.1.7. Cases caused by this variant have been reported in the US at the end of January 2021.
- In Brazil, a variant called P.1 emerged that was first identified in travelers from Brazil, who were tested during routine screening at an airport in Japan, in early January. This variant contains a set of additional mutations that may affect its ability to be recognized by antibodies. This variant was first detected in the US at the end of January 2021.
These variants seem to spread more easily and quickly than other variants, which may lead to more cases of COVID-19. An increase in the number of cases will put more strain on health care resources, lead to more hospitalizations, and potentially more deaths.

Recent data from the Netherlands and France suggest that of the patients with coronavirus who are admitted to intensive care units (ICU), 30-70% develop blood clots in the deep veins of the legs, or in the lungs. Pathophysiology of covid19 : (18)
The lungs are the organs most affected by COVID-19 because the virus accesses host cells via the enzyme angiotensin-converting enzyme 2 (ACE2), which is most abundant in type II alveolar cells of the lungs. The virus uses a special surface glycoprotein called a “spike” (peplomer) to connect to ACE2 and enter the host cell. The density of ACE2 in each tissue correlates with the severity of the disease in that tissue and some have suggested that decreasing ACE2 activity might be protective, though another view is that increasing ACE2 using angiotensin II receptor blocker medications could be protective and that these hypotheses need to be tested. As the alveolar disease progresses, respiratory failure might develop and death may follow. (18)
The virus also affects gastrointestinal organs as ACE2 is abundantly expressed in the glandular cells of gastric, duodenal, and rectal epithelium as well as endothelial cells and enterocytes of the small intestine. (18)
The expanding part of the lungs, pulmonary alveoli, contains two main types of functioning cells. One cell, type I, absorbs from the air, i.e. gas exchange. The other, type II, produce surfactants, which serve to keep the lungs fluid, clean, infection-free, etc. COVID-19 finds a way into a surfactant-producing type II cell and smothers it by reproducing the COVID-19 virus within it. Each type II cell that perishes to the virus causes an extreme reaction in the lungs. Fluids, pus, and dead cell material flood the lung, causing the coronavirus pulmonary disease. (18)
Angiotensin-converting enzyme (ACE):
ACE is a central component of the renin–angiotensin system (RAS), which controls blood pressure by regulating the volume of fluids in the body. It converts the hormone angiotensin I to the active vasoconstrictor angiotensin II. Therefore, ACE indirectly increases blood pressure by causing blood vessels to constrict. ACE inhibitors are widely used as pharmaceutical drugs for the treatment of cardiovascular diseases. The enzyme was discovered by Leonard T. Skeggs Jr. in 1956. It is located mainly in the capillaries of the lungs but can also be found in endothelial and kidney epithelial cells. (23)

ACE2 is known to be present in most organs. ACE2 is attached to the cell membrane of mainly lung alveolar epithelial cells, enterocytes of the small intestine, arterial and venous endothelial cells, and arterial smooth muscle cells in most organs. ACE2 mRNA expression is also found in the cerebral cortex, striatum, hypothalamus, and brainstem. (22)
Angiotensin Receptor Blocker (ARBs): (21)
Angiotensin II is a very potent chemical formed in the blood that causes muscles surrounding blood vessels to contract, thereby narrowing the vessels. This narrowing increases the pressure within the vessels and can cause high blood pressure (hypertension). Angiotensin II receptor blockers (ARBs) are medications that block the action of angiotensin II by preventing angiotensin II from binding to angiotensin II receptors on the muscles surrounding blood vessels. As a result, blood vessels enlarge (dilate), and blood pressure is reduced. Reduced blood pressure makes it easier for the heart to pump blood and can improve heart failure. In addition, the progression of kidney disease caused by high blood pressure or diabetes is slowed. ARBs have effects that are similar to angiotensin-converting enzyme (ACE) inhibitors, but ACE inhibitors act by preventing the formation of angiotensin II rather than by blocking the binding of angiotensin II to muscles on blood vessels. What are the uses for ARBs? (21)
ARBs are used for controlling high blood pressure, treating heart failure, and preventing kidney failure in people with diabetes or high blood pressure.
ARBs also may prevent diabetes and reduce the risk of stroke in patients with high blood pressure and an enlarged heart.
ARBs also may prevent the recurrence of atrial fibrillation.
Since ARB medications have effects that are similar to those of ACE inhibitors, they often are used when ACE inhibitors are not tolerated by patients (for example, due to excessive coughing). (21)
What are the side effects of ARBs? (21)
ARBs are well tolerated by most people. The most common side effects are

Compared to ACE inhibitors, cough occurs less often with ARBs.
Serious side effects of ARBs
The most serious, but rare, side effects are
kidney failure,
Liver failure (hepatitis),
Serious allergic reactions,
A decrease in white blood cells,
A decrease in blood platelets, and
Swelling of tissues (angioedema).
There have been reports of rhabdomyolysis (destruction of skeletal muscle) in patients receiving ARBs.
Individuals who have narrowing of both arteries that supply the kidneys or have had a severe reaction to ARBs should avoid them.
Like other antihypertensives, ARBs have been associated with sexual dysfunction. (21)
Cardiovascular diseases and/or their therapy, by affecting ACE2 levels, may play a pivotal role with regard to infectivity and outcome of COVID‐19. Whether treatment or disease-induced upregulation of ACE2 influences the course of COVID‐19 urgently needs to be determined. (24)
Link Between Vitamin D Deficiency and COVID19 Deaths
A team of Indonesian researchers led by Prabowo Raharusuna released a working paper showing a significant association between COVID19 deaths and vitamin D insufficiency or deficiency in a sample of 780 laboratory-confirmed cases of SARS-CoV-2.
“This study focused on identifying patterns of mortality among patients infected with COVID19 and the possible association between serum 25(OH)D [vitamin D] levels and mortality outcomes,” state the researchers.
The key findings are as follows:
A majority of COVID19 cases with vitamin D deficiencies died.
The odds of death were higher in older and male cases with preexisting conditions and lower than normal Vitamin D levels.
When accounting for the effects of age, sex, and comorbidity, vitamin D status is strongly related to COVID19 mortality. (28)
Another Causes of Vitamin D Deficiency
- You don’t consume the recommended levels of the vitamin over time. This is likely if you follow a strict vegan diet, because most of the natural sources are animal-based, including fish and fish oils, egg yolks, fortified milk, and beef liver.
- Your exposure to sunlight is limited. Because the body makes vitamin D when your skin is exposed to sunlight, you may be at risk of deficiency if you are homebound, live in northern latitudes, wear long robes or head coverings for religious reasons, or have an occupation that prevents sun exposure.
- You have dark skin. The pigment melanin reduces the skin’s ability to make vitamin D in response to sunlight exposure. Some studies show that older adults with darker skin are at high risk of vitamin D deficiency.
- Your kidneys cannot convert vitamin D to its active form. As people age, their kidneys are less able to convert vitamin D to its active form, thus increasing their risk of vitamin D deficiency.
- Your digestive tract cannot adequately absorb vitamin D. Certain medical problems, including Crohn’s disease, cystic fibrosis, and celiac disease can affect your intestine’s ability to absorb vitamin D from the food you eat.
- You are obese. Vitamin D is extracted from the blood by fat cells, altering its release into the circulation. People with a body mass index of 30 or greater often have low blood levels of vitamin D. (29)
CLINICAL CLASSIFICATION (from Pathological point of view)
Mild to moderate cases
- Fever, nasal congestion, dry cough, fatigue, sputum formation, shortness of breath, sore throat, headache, body ache, chills, nausea, vomiting, diarrhoea, loss of smell and taste etc.
- This constitute 80% of patients.
- The difference in mild and moderate cases is that the above symptoms are mild in the former and there is no pneumonia while in the latter the symptoms are more intense and manifestations of pneumonia are seen in imaging.
Severe cases
- Adults who meet any of the following criteria: respiratory rate; > 30 breaths/min; oxygen saturations; 93% at a resting state; arterial partial pressure of oxygen (PaO2)/oxygen concentration (FiO2); < 300 mmHg. Patients with > 50% lesions progression within 24 to 48 hours in lung imaging should be treated as severe cases.
- Pneumonia with fluid accumulation
- This constitutes 14% of patients.
Critical cases
- Extensive pneumonia
- Meeting any of the following criteria: occurrence of respiratory failure requiring mechanical ventilation; presence of shock; other organ failure that requires monitoring and treatment in the ICU.
- This constitutes 6% patients. (15)
Liver damage:
Some patients hospitalized for COVID19 have had increased levels of liver enzymes — like alanine aminotransferase (ALT) and aspartate aminotransferase (AST) — that indicate their livers are at least temporarily damaged. Also, liver damage is more common in patients who have severe COVID19 disease. (30)

Antibody and the latent state of COVID-19
These antibody tests will be able to measure that level of serology presence, that level of antibodies, but that does not mean that somebody with antibodies” is immune, said Dr. Maria Van Kerkhove, head of WHO’s emerging diseases and zoonosis unit. So-called serological, or antibody, tests can indicate whether a person has had Covid-19 in the past and was either asymptomatic or recovered. (26)
Miasmatic assessment of COVID-19 cases
If we study the coronavirus disease in evolutionary totality, it is possible to see the cleavage. We can split the day wise data, analyse the pathology which is supervening and assess the miasmatic state. This will help us figure out the remedies.
Psoric manifestations
Mild symptoms such as fever, cold, throat pain, sore voice, headache, diarrhoea etc. Return to baseline of health is early and easy. Immunity good. Good response to the treatment. No complications. COVID-19 is limited to upper respiratory tract. Psora applies to Day 1-4.
Sycotic manifestations
The mild symptoms in psora become intense and constant. Lingering response. Weakness and dullness. Fever becomes constant and heaviness of head. Dry cough in bouts, often with expectoration; thick, yellow-greenish. Soreness of voice increases. Joint pains. Yet COVID-19 is limited to URT. Sycotic miasm is mostly for Day-5 status of the patients. 80% mild to moderate cases will be under the dominance of psoric and sycotic miasms.
Tubercular manifestations

There are two types of patterns of response: 1. Acute, sudden onset and rapid pace of the disease. 2. Slow, then rapid pattern. COVID-19 usually follows the second pattern. It is from the Day 6 that we are able to see the tubercular miasmatic dominance and up to day 8.
This phase is characterized by high grade and persistent fever, intense throat pain and development of pneumonia and pleural effusion. Intense heaviness in chest, breathlessness, increased respiration, low oxygen saturation, enteritis, profound debility and toxic appearance are on the screen.
The type of severe disease of 14% of patients is due to the dominance by the tubercular miasm. Day 6-8 needs careful monitoring and a remedy selected on the basis of totality and that covers the miasmatic state, if given in the right posology is able to help the system from the clutches of death. There are three signals that indicate the increasing influence of tubercular miasm. Very severe throat pain (probably a last-ditch effort on the part of the organism (through throat) to prevent the virus to go down in the lungs), pneumonia which is rapidly increasing and lymphopenia (a critical factor associated with disease severity and mortality).
The three stages of pneumonia represent the tubercular miasm. Don’t misunderstand that the first stage is psoric. It is not only replication but rapid changes of destruction (tubercular miasm is a blend of hyperreactivity of psora and destruction of syphilis).
Syphilitic manifestations
No response to treatment. Disease progresses with high pace. Complications à, hemodynamic state poor. Cyanosis. Kidney failure. Collapse. Comatose condition. Respiratory collapse. Multi-organ failure.
The above miasmatic criteria should be applied to all cases of pandemic COVID-19 for preventive and treatment modality. If we take into account the clinical features of COVID-19 in totality, the tubercular miasm is preponderant over the system right from the Day-4, and if the clinical condition becomes worse, it progresses to the syphilitic miasm.
The most common cause of death is respiratory failure. Other causes of death are pneumonia (leading to sepsis), high fever (leading to neurological problems), dehydration and electrolyte imbalance (from excessive vomiting and diarrhea).

Obesity:
Some coronavirus patients with obesity may already have compromised respiratory function that preceded the infection. Abdominal obesity, more prominent in men, can cause compression of the diaphragm, lungs, and chest capacity. Obesity is known to cause chronic, low-grade inflammation and an increase in circulating, pro-inflammatory cytokines, which may play a role in the worst Covid19 outcomes. (25)

How to avoid catching or spreading coronavirus

- Wash your hands with soap and water often – do this for at least 20 seconds
- Always wash your hands when you get home or into work
- Use hand sanitiser gel if soap and water are not available
- Cover your mouth and nose with a tissue or your sleeve (not your hands) when you cough or sneeze
- Put used tissues in the bin immediately and wash your hands afterwards

The risk of death is lower still, at roughly 0.8%. This means the overall likelihood of recovery is 99.2%, but that is misleading. The death rate is highly concentrated- almost 98% concentrated- among those over age 70, and further concentrated among those over age 80. 2) Gelsemium sempervirens: in covid19

4) Eupatorium perfoliatum: in covid19
Coryza with sneezing; aching in every bone. Sharp pain through right chest on deep inspiration. Tremendous aching as if the bone are broken.
Cough with soreness of chest, from laryngeal tickling; it hurts head and must hold the chest amel. Influenza with high fever, great soreness & aching of bones. Great thirst for cold drinks during the chill or even during the rigor.

Currently, no specific prophylaxis or treatment is available for CoV infections and therefore we investigated the antiviral potential of Echinacea purpurea (Echinaforce®) against human coronavirus (HCoV) 229E and the highly pathogenic MERS- and SARS-CoVs in vitro. We found that HCoV-229E was irreversibly inactivated when exposed to Echinaforce at 3.2µg/ml IC50. Pre-treatment of cell lines, however, did not inhibit infection with HCoV-229E and post-infection treatment had only a marginal effect on virus propagation at 50 µg/ml. However, we did observe a protective effect in an organotypic respiratory cell culture system by exposing pre-treated respiratory epithelium to droplets of HCoV-229E, imitating a natural infection. Finally, antiviral activity was not restricted to common cold coronaviruses, as the highly pathogenic SARS- and MERS-CoVs were inactivated at comparable concentrations. These results suggest that Echinacea purpurea preparations, such as Echinaforce, could be effective as prophylactic treatment for all CoVs, including newly occurring strains, such as SARS-CoV-2. (13)
